Abstract

Background

The quantification of the flow returning from the head through the cervical veins and the collaterals of the internal jugular vein (IJV), is becoming of prominent interest in clinical practice. We developed a novel model to calculate the cerebral venous return, normalized to the arterial inflow, in the different segments of the IJV.

Methods

We assessed, by established Echo Colour Doppler (ECD) methodology, the head inflow (HBinF) defined as the sum of common carotids and vertebral arteries, as well as the cerebral flow (CBF) defined as the sum of internal carotid and vertebral arteries. We also assessed the head outflow (HBoutF) defined as the sum of the measurements at the junction of the IJV and the vertebral veins. In addition, we also calculated the collateral flow index (CFI) by estimating the flow which re-enters directly into the superior vena cava as the amount of blood extrapolated by the difference between the HBinF and the HBoutF. We preliminarily tested the model by comparing ten healthy controls (HC) with ten patients affected by chronic cerebral spinal venous insufficiency (CCSVI), a condition characterized by some blockages in the IJV which are bypassed by collateral circulation.

Results

In HC the HBinF was 956+-105ml/min, whereas the HBoutF was > 90% of the HBinF, leading to a final CFI value of 1%. The last result shows that a very small amount of blood is drained by the collaterals. In upright we confirmed a reduction of the outflow through the IJV which increased CFI to 9%. When we applied the model to CCSVI, the HBinF was not significantly different from controls. In supine, the flow of CCSVI patients in the IJV junction was significantly lower (p < 0.001) while the correspondent CFI value significantly increased (61%, p < 0.0002).

Conclusions

Our preliminary application of the novel model in the clinical setting suggests the pivotal role of the collateral network in draining the blood into the superior vena cava under CCSVI condition.

Conclusion

We developed a new model that permits a detailed ECD quantification of the cerebral venous return, including an estimation of the amount of blood flowing from the collaterals to the caval system or to the IJV. The preliminary application of the model seems to indicate how a significant rate of the head inflow is drained by the collateral network rather than by the IJV in the CCSVI condition. This may help the interpretation of several findings assessed with different techniques, where it was not possible to assess the outflow contribution of the collateral network, as well as the rate of the inflow going in the main venous paths. For instance, the higher flow in the collateral network may explain the longer cerebral circulation time measured by means of contrast-enhanced US, as well as the slower discharge and increased resistance measured in MS [32-34]. Our preliminary report needs to be further corroborated by reproducibility analysis, wider number of subjects and pathological conditions, and possibly, by a multicenter design. This may lead to a further advancement for the circulatory quantification of the CCSVI condition in the clinical setting via ultrasonography.

很困擾MS人的一個問題 :尿失禁 生活品質是首要考量

其實多數的多發性硬化症(MS)病患有中到重度的排尿問題,但是他們未接受適當的評估或治療,然而,以現有藥物其實可以輕易地處置這個狀況。

有哪些排尿問題呢?

MS和泌尿症狀的統計是重要的。有此疾病的400,000名美國人中,75%報告有尿失禁,50%至80%有排尿障礙。MS病患的男女比率為1比2,有十分之一MS病患的第一個症狀就是尿滯留

泌尿失能的特定問題,包括排尿頻率、急尿頻率、覺得急尿時的漏尿、夜間起床尿尿的頻率等。

特別是夜間起床尿尿的頻率太長(一夜七次郎以不足為奇,有病友一夜9次郎)。試著想像,若你一夜起來7次,這些會導致嚴重的睡眠不足。你白天還能夠上班嗎? 整個人的累,而累只會導致神經持續的惡化,甚至發作。

這的確是一個非常嚴重的問題,但是傳統的治療方式不外乎以下:

藥物治療:給你藥吃就對了(但是很多藥物副作用都會導致更嚴重的便秘,MS人已經有嚴重的便秘問題了,吃了反而加重,很傷腦筋,相信我,我都吃過。)
復健治療:做做骨盆肌肉的訓練和復健(相信我,我也都做過)

但是成效有限,一覺到天亮,已經成為我的夢想。(微不足道的夢想)


有病友在facebook討論區提到了肉毒桿菌注射治療膀胱過動的問題後,我上網搜尋一下給大家分享。發現一項發表于2012年7肉毒桿菌月3日Neurology雜志的新的研究發現,A型肉毒毒素可能有助于減輕多發性硬化(MS)患者出現的手臂部震顫。

A型肉毒杆菌毒素,也稱保妥適,已迅速從一種抗衰老治療中衍生出其他功能,用于治療慢性偏頭痛和出汗過多。其也可用于治療MS的某些症狀,如膀胱過動症和手臂肌肉痙攣。

研究者對23例MS患者注射A型肉毒毒素或生理鹽水安慰劑3個月,並對治療前,治療中以及治療後患者震顫嚴重程度以及書寫能力進行了測量。每6周進行一次視頻評估,共爲期6個月。

研究發現,與安慰劑相比,肉毒毒素治療6周以及3個月後,患者的震顫嚴重程度和書寫能力均有顯著改善。在震顫嚴重程度方面,參與者平均提高了2分,震顫程度從而達到中度到輕度。在書寫能力方面,參與者平均提高了1分。

接受肉毒毒素治療的患者中,42%出現肌無力狀況,而在安慰劑組爲6%。但這些肌無力狀況一般表現溫和,可在2周內消失。

來自澳大利亞皇家墨爾本醫院的首席研究員Anneke van der Walt博士表示,大多數患者能非常良好地耐受這種注射劑,並且他們一旦看到從中得到的好處就希望能繼續這種治療。

附上病友的經驗(非常寶貴的經驗)

病友多是因神經受損導致膀胱餘尿多頻尿or尿不出來,打肉毒桿菌可放鬆膀胱肌肉,有效改善"尿"的問題,但是藥效過了就得再施打藥效也因人而異,我可以撐2~3年

之前 頻尿,容量不到 100CC,又忍不住,所以出外都是一直找廁所,導致不敢喝水,治療之後,容量450CC,可以忍尿,但是超過這量就會有一點漏尿,所以覺得脹尿時去上廁所壓壓就可以解出大概150~200CC餘尿200~250就不覺得有尿意,但是晚上就必須導尿。

多發性硬化症一般都是膀胱跟括約肌無法配合,如果是尿不出來的就打在膀胱頸括約肌這要經由醫師評估

我發病已經21年了但是常常為了整夜上廁所上到發脾氣,也不敢出門,所以才會去打肉毒桿菌,這問題解決了情緒也變得比較開朗。

12年前打第一次兩單位花蓮慈濟泌尿科郭漢崇主任當時研究用所以不用錢,後來陸續都有打,但是自費,今年5月底又去,郭醫師說本來去年就應該通過健保但是國健局認為兩單位太浪費,郭醫師幫我打1單位,效果依樣很好,郭醫師在新店慈濟有門診,由別的醫師操刀,有需要的病友可以先去門診評估,郭醫師人很好,記得這資訊我10多年前曾PO上協會呀當時也是花蓮病友心慈幫我,生活品質變好了。

目前還沒通過健保給付,聽說是劑量上的問題,美國是2單位台灣健保主張不要浪費只要1單位,郭醫師說我是很好的例子1單位就有很好的效果,纖維化我倒是沒考慮,不過出外不用到處跑廁所,生活品質情緒都變得更好。

找泌尿科醫師,因為我們的病一般醫師不建議半身麻醉,所以我都是全身麻醉經由尿道打肉毒桿菌劑量是由醫師決定,次數是體質決定,我可以撐滿久,有人只有半年就須重打,肌無力倒是沒有,因為是打在膀胱他不會吸收到全身,泌尿科醫師也不是每個都會這手術

α-腎上腺作用劑 (Alpha-adrenergic agonists)

本類藥品藉由刺激尿道平滑肌收縮,短期用於混合型尿失禁治療,然而可能造成殘餘尿量增加,而導致應力性尿失禁惡化。這類藥品常用品項為imipramine。Imipramine原本屬於三環抗焦慮藥物,具α腎上腺作用和抗膽鹼作用,不建議使用於合併認知障礙且正使用膽鹼性藥物治療的老年族群。Imipramine主要副作用是全身倦怠、嗜睡、口乾及姿態性低血壓。Imipramine不能和單胺氧化?抑制劑同時服用,以避免癲癇及少見的血清素症候群(高血壓,高熱,肌陣攣,精神狀態改變)發生。長期服用imipramine後,如果需要停藥則需採取漸減方式停藥。


β3-腎上腺接受體作用劑(Beta 3-adrenergic receptor agonists)

第一個β3腎上腺接受體作用劑 “mirabegron” 於2012年由美國FDA核准上市使用於膀胱過動症治療,台灣目前則尚無輸入。主要控制尿失禁的機轉是透過刺激膀胱壁上β3 -腎上腺接受體,促使膀胱鬆弛,增加尿量儲藏量。目前僅有少數臨床研究結果顯示其可減少排尿頻次,常見副作用包括高血壓、鼻咽炎、尿道感染、便秘、疲倦、心搏過速和腹痛,因此對於血壓控制不良者不建議使用這類型藥物。

Duloxetine

Duloxetine是一種選擇性針對serotonin及norepinephrine 再吸收抑制劑,在歐洲其可用於應力性尿失禁治療,主要機轉乃藉由刺激會陰運動神經元α腎上腺素和5 - 羥色胺受體(5-hydroxytryptamine-2- receptor),來改善排尿症狀,然而美國FDA尚未核准此適應症,台灣FDA亦是。噁心是duloxerine常見的不良反應(23%),雖然連續服用數週後症狀可逐漸獲得改善,然臨床試驗結果顯示,約20%的人會因藥物不良反應而停止此藥物治療,另duloxerine不建議使用於慢性肝炎患者。

Estrogen

一系統性研究結果顯示口服estrogen可能會惡化尿失禁問題,然而局部使用estrogen則可能可以改善尿失禁症狀,但目前並無明確的證據3。

α-腎上腺作用抑制劑 (Alpha-adrenergic antagonists)

一小型隨機研究結果顯示,有排尿困難合併低排尿量的女性病人,每日服用0.2 mg的tamsulosin 達六星期,約35%的人可獲改善4。α-腎上腺作用抑制劑可能適用於上薦骨脊髓受損(suprasacral spinal cord injured)或多發性硬化症患者(multiple sclerosis)。食物會影響本藥物的吸收,建議每日於固定時間飯後30分鐘服用。

肉毒桿菌素A型 (Botulinum toxin A)請大家多注意這裡。

膀胱過動症(Overactive bladder syndrome,OAB) 是一群集性的症狀,包括尿急,頻尿,急迫性尿失禁和/或夜尿,主要與逼尿肌過度活動,逼尿肌不自主收縮有關。Botulinum toxin A具抗膽鹼作用,於逼尿肌處局部注射,對頑固性急迫性尿失禁及逼尿肌過度活動者約69%的人可能可以獲得改善,然26% - 43%的病人於治療後4-6星期可能發生尿液滯留的問題,而需藉助導尿來幫助排尿。下泌尿道局部注射Botulinum toxin可能有感染之虞,發生率介於6-35%間,Botulinum toxin導致的全身性副作用並不多,但並不排除Botulinum toxin會由注射部位擴散至全身,當病人有吞嚥困難,眼瞼下垂,頭困難,腿無力和麻木時,需考量此可能性5,6,7。


結論

尿失禁的治療方式主要採階梯方式,由改變生活方式開始,降低每次飲水量,特別是睡前幾小時,並避免食(飲)用含咖啡因和辛辣的食物,若患者有過重的問題,則需進行減重,隨後並給予行為及藥物治療,對於上述無效之嚴重病人則予以手術治療。臨床上對於尿失禁的治療藥物大多數是針對逼尿肌不穩定所造成的尿急,尿失禁或是所謂膀胱過動症的治療,目前針對應力性尿失禁尚無理想用藥,duloxetine也許可以提供些許療效,但部分病人對本藥耐受性並不好。過往認為雌激素對改善更年期相關症狀有幫助,但近期研究分析發現口服雌激素可能會惡化尿失禁,而雌激素局部使用則反而可能可以改善病人失禁的症狀。

3年後的病友追蹤

有很長一段時間, 世界上仍舊爭論著靜脈擴張的成功率或準確度等等等....

先把這些拋開, 看看當初2010年做了手術後的病友情形如何?

那我呢?

我, 目前有了工作. 已經持續一年, 比較不那麼疲累, 雖然仍受著許多後遺症之苦 ,那也是因為狹窄的地方實在太過棘手, 整體而言, 我給予高度正面的評價.

多發性硬化症在中國



新京報訊 (記者 潘波)5月29日是第五個世界多發性硬化日。據介紹,全世界約有250萬多發性硬化患者。北京市天壇醫院神經內科張星虎教授在多發性硬化關愛月的活動中強調:“多發性硬化是一種無法治愈的神經系統進展性疾病,易發於15-40歲青壯年人群,女性多發性硬化患者大約是男性患者的2-3倍。”
目前,我國已確診的多發性硬化症患者在3萬左右,但因為多發性硬化症的症狀特徵不明顯,容易與其他疾病相混淆、易誤診。“多發性硬化診療的關鍵在於早期診斷及早期規範治療。

潛在+誤診的有?

多發性硬化診療的關鍵在于早期診斷及早期規范治療。中國大多數患者、親屬甚至部分醫務工作者對多發性硬化及其治療缺乏足夠的認識,專科診治多發性硬化的診療中心及專家國內數量還不多,加之此病初期不容易與其它神經科疾病鑒別,相當多的患者貽誤了治療的良機。“患者如果早期能夠獲得正確的診斷,多發性硬化症是可以治療的。”但胡學強也坦言,由于能夠延緩殘疾進展的有效治療藥品如β-幹擾素依賴進口,價格較高並且沒有被醫療保險覆蓋,使中國的多發性硬化患者較國外患者發展為殘疾的比例要高。有的患者可能還會受到一定程度來自家庭、社會的歧視,甚至因此失去工作、家庭,進而對其生活造成更大影響。此外,由于該病久治不愈,患者容易引發抑鬱等心理疾病,導致其自殺率是正常人群的7.5倍。
身為廣東省政協委員的胡學強曾連續三年提交提案,呼吁將多發性硬化症的診治納入國家醫療保障係統。他強調,這是關愛多發性硬化症患者的重要方式。
會議透露,國家有關部門也在多方了解多發性硬化症及其發病人群的情況。今年底或明年初可能會出臺相關政策,幫助多發性硬化患者解決部分難題。

【記者賴永隆/台南報導】中華醫事科大22日頒發教育部獎勵身心障礙學生奮發向學特殊教育獎及助學金,共112位學生受獎;罹患多發性硬化症的陳怡穎,不僅全身刺痛,在視力聽力受損下,仍奮發向學,樂觀進取。
陳怡穎罹患多發性硬化症,發病時全身刺痛、感覺麻木、失去平衡感,有時無法看清眼前物體,有時耳朵感覺有異物塞住影響聽力,她每天需注射針劑,但怡穎仍不畏病痛之苦,努力學習。
除學業優異外,2011年曾獲新台南美食番薯王創意料理賽大專組冠軍,去年又獲得台南米食創意料理第3名及台南西瓜節西瓜料理校園組季軍。
頒獎表揚典禮邀請了全校各單位主管、全體身心障礙學生、班導、輔導老師及志工200餘人參加,為不向命運低頭、表現優秀的學生們加油鼓勵。◇

美國隊長之再論類固醇

最近, 如同過去的日子一般, 是季節交替的時節,
春夏之際, 溫度變化劇烈, 熱對流旺盛,
濕度驟昇, 雨滂沱的下
身體一定很難受

從facebook的病友社群裡看到一個個進了醫院
心想; 又是瘟疫蔓延的季節

由記得去年前的此時附近, 也回醫院進場維修保養

然而身體細胞的記憶似乎忘記不了那過去深深的烙印著的傷痕
(5年內就有4次復發就是在5月和六月之間)
就在幾天前, 我隨著細胞喚醒的記憶, 回到了高醫的急診室.

在急診室和醫師討價還價, 就是類固醇的劑量
住院能不能打少一點的劑量?
要住院就要打500mg 或 1000mg (哪有這樣的道理)
書上都是這樣....(人是活的,書是死的,人都要因材施教, 劑量當然也可以因人而異啊)
我以前在北榮和新竹都有打過100mg的 恢復也不錯
我不要overdose 而且我的身體無法承受如此高的劑量
給我100mg 我再轉口服
能否住院觀察?
上面的不太能講喔, 要就要500mg 或 1000mg  (這是不變的教條嗎?)
再三溝通後, 就急診室給了100mg,
瞬間又活過來了, 只不過我心理自知, 這只是短時間的好光景,
需要更多的時間休息和恢復
注射之後的每天早上一定都會想睡覺

這次的反應比上次好多了, 或許也是好轉的跡象.
希望之後再也不要有這樣的復發和細胞記憶

祝大家平安度過這個季節
也希望醫師們不要用固定的配藥方式,請仔細觀察病人, 按照病人的需求給予適當的劑量
以免用藥過量而傷害了病人寶貴的身體


以上, 也是我自己經過這幾年下來的嘗試和經驗, 不一定適用每個人,
猶記的第一次, 1000mg注射3天後就產生了幻覺和心悸胸悶的現象, 也許就是身體無法承受這樣高的劑量的反應吧.無論如何, 用藥還請盡量謹慎小心.

在 了生兩個孩子之後, 他的生活因為MS受到了莫大的改變, 從復發緩解型變成了2次逐漸退化型的診斷中, 力行著在TED演講的內容, 長期使用著飲食控制, 讓他的生活逐漸改變. 影片中可以看到美國飲食的型態
. 在逐漸西化的飲食當中, 若不想連疾病都西化, 請盡量的做到好的飲食習慣.

B1, B6, B12, creatine(肌酸) ,魚油, 蔬果 是他所建議的.  我也建議能夠由天然的食物食用最好.

少肉,.少吃油炸的食物, 保持良好的生活習慣. 永遠不算晚.


自我經驗分享 
 
長期下來, 因神經受損而導致的胃腸蠕動障礙會有很多影響層面, 給同樣疾病的人參考. 便秘脹氣, 是很常見的情形, 若沒有,恭喜您, 若有, 要好好的注意. 
這段時間中, 我感受到若腸子累積了大量的氣體以及糞便, 將會對下腹部以及下肢的血管造成壓迫的可能性.
當壓迫造成的時候, 下肢將會感到沉重, 僵硬,和更麻. 甚至可能造成血液不循環而導致頭部的缺氧, 而昏昏欲睡.... 這種想睡的感覺像極了我之前發病後沒多久的情形(特別容易累, 尤其是早上9:30後~10:30).
因為之前自我檢查中顯示了自己存在有CCSVI的情形, 而桑柏尼的論文和病友中也發現有部分的病人存在有Lumber vein 和iliac vein狹窄的問題. 所以也間接的說明為何我通常在未排便一段時間後, 下肢將會感到沉重, 僵硬,和更麻的情形,甚至想睡. 
而當糞便排出後, 症狀立刻好轉, 頭腦也立刻清醒....

胃腸,首重之物.
